• Each player will be expected to complete a football equipment issued form when equipment is issued
• All fees are due at the time equipment is issued
• Equipment handouts isn’t a quick process so please allot enough time for us to properly fit your player for equipment

USA FOOTBALL RECOMMENDATIONS REGARDING HAIRSTYLE: Your player should be fitted for their helmet with the hairstyle he will wear while at practices and games. Helmet fit can change if your players hairstyle changes. For example, a long-haired athlete who gets a very short haircut may need to adjust the fit of the helmet.

• Youth size Jersey fee $40.00
• Adult size Jersey fee $50.00
*******Note please bring your jersey from last year to try on with the shoulder pads to insure it fits properly

SALES PAYABLE TO SHIVELY SPORTING GOODS:
SHIVELY WILL BE ON SITE 7-20-15 ONLY SELLING MISCELLANEOUS FOOTBALL ITEMS TO ST. MARTHA PLAYERS AT A TEAM RATE.
******NOTE CHECKS TO SHIVELY WILL BE MADE PAYABLE TO THEM. PLEASE BRING A SECOND CHECK IF YOU PLAN ON BUYING SOMETHING FROM SHIVELY

The CSAA has adopted the USA Football’s HEADS UP program that the NFL endorses. Your player’s safety is our priority at St. Martha and with that in mind, much of the equipment has been reconditioned so our players are safely and properly equipped. Many different player safety systems have been put in place this year at St. Martha and your coaches will share some of those systems with your players and you parents.

A successful Sports Ministry program requires the contributions of many! If you are interested in helping with the St. Martha Sports Ministry program for the 2015-2016 school year, please complete this form and return to me through school, Attn: Cynthia Hatfield c/o Harrison Hatfield 4D. You may also contact me via email or my cell. Plans for the new school year and athletic season will start this spring.

Even if you are not athletically inclined, there are several areas in which parishioners can make a valuable contribution to the success of the St. Martha Sports Ministry program. Some of the positions we need to fill are listed below. However, if you would like to contribute in an area that is not listed below, please contact me to discuss your interest to get involved.
